IRS Form SS-8 is the form that either employers or employees may use to report on, or request a ruling as to the proper classification status of a worker.
IRS Form 4852 is a form that is used by practitioners advising employees when an employer refuses to correct or amend an erroneous Form W-2, by and through issuance of a Form W-2c.
