IRS Form 3949-A, Information Referral, which is the form recommended for use when you suspect or know of an individual or business that is not complying with the tax laws on issues such as:
False Exemptions
Deductions
Kickbacks
False/Altered Documents
Failure to Pay Tax
Unreported Income
Organized Crime
Failure to Withhold
IRS Form 14039 is the form to be used if you suspect someone stole your identity and used your SSN for employment purposes or could use your SSN or file a tax return.
IRS Form 14157 is the form to be used if you suspect fraudulent activity or an abusive tax scheme by a tax return preparer or tax preparation company.
IRS Forms 14157 and 14157-A are used if you suspect a tax return preparer filed a return or altered your return without your consent and you are seeking a change to your account.
IRS Form 14242 is the form to be used if you suspect an abusive tax promotion or promoter.
IRS Form 13909 is the form to be used if you suspect misconduct or wrongdoing by an exempt organization or employee plan.
The IRS also has an interest in hearing from you if you suspect you have received or are aware of fraudulent IRS e-mails and websites.
